REQUIRED: Upload proof of condition or medical documentation. This can be a photo of a valid dispensary card (NOT general health card / Care card), prescription or prescription bottle, or any other medical documentation where your name is clearly shown.

REQUIRED: Upload secondary ID with your address (such as a health card, non-vehicle license, or addressed envelope from a phone or utility company) for verification. This does NOT need to include a photo, but it MUST show your address for delivery purposes.
This identification must NOT be a duplicate of the identification submitted in the Photo ID section above. We require two different forms of identification.
